A very effective way to remove grass and weeds is to cover the area with a layer of cardboard and put a couple of inches of compost or mulch over it. Wait six months, then rototill the area, amend your soil as needed, and plant. Landscaping cloth will help with weeds for the first year or so, but after a while new weeds will just seed over it. It is best to plant thickly, use groundcover, mulch one or two times a year, and choke out the weeds. After the garden is established, weeds become less and less of a problem.
Boil 1/2 gallon of water
Add 1/4 cup salt
Add 1/2 cup plus 2 tablespoons white vinnegar
Pour solution directly on weeds in driveways, pathways, sidewalks, etc., while still hot.
